- Accomplished Admission Form with a 1×1 ID picture
- Entrance Examination result from the Student Assistance Center and an interview with the Dean of College
- Form 138 (Report Card), PSA Birth Certificate, and Baptismal Certificate (Original and Photocopy)
- Certificate of Good Moral Character
For Transferees:
- Informative copy of Transcript of Records (TOR) and Honorable Dismissal
- Certificate of Good Moral Character
- PSA Birth Certificate and Recent ID Pictures
