- Accomplished Admission Form with a 1×1 ID picture
- Entrance Examination result from the Student Assistance Center and a formal interview with the Dean of College
- Form 138 (Report Card) for new students or an informative copy of the Transcript of Records for transferees
- PSA Birth Certificate and Baptismal Certificate (Original and Photocopy)
- Certificate of Good Moral Character
